We Care
Our Practice Administrator, Leana Coradini, is here to direct your needs. She leads a staff of support professionals who undergo continuing education and training.

We work with patients and their primary care physicians to help evaluate, recommend and treat medical conditions related to the urinary tract of both sexes and to the male reproductive system. Our treatments are comprehensive and cost-effective.

We provide comfortable and confidential environments in which patients and family members are encouraged to ask questions. For your convenience, Old Pueblo Urology has five offices throughout Tucson and Green Valley.

First Visit
Old Pueblo Urology is now accepting new patients. On your first visit please bring the following information with you.

• Insurance card (a copy of which is required at each office visit)
• Photo ID
• Patient information forms (if any were previously mailed to you)
• Prior medical records
• Referral form if required by your insurance provider
• Your co-payment if applicable
• A list of current prescriptions and/or over-the-counter medications you are taking, including dose and frequency
• Recent x-rays
• Allergy information

Referrals
HMO Referrals are the responsibility of you and your primary care physician.

Payment
The bookkeeping department is here to help you with any financial or insurance plans. Fees are payable a the time of each office visit unless prior arrangements have been made. It is our policy to treat those in need. If you must make financial arrangements, please discuss your needs with our billing department.

Insurance
We accept many insurance plans. If your insurance requires authorization for an office visit, please inform us when you make an appointment. We participate with all commercial insurance carriers, Medicare, Tricare and many HMOs. As a service to our patients, our staff will file your primary insurance claims—including secondary payers if necessary. You are responsible for full payment on any deductible, co-payment, or percentage of the fee required by your insurance policy.

Prescriptions
Routine requests for prescription refills should be called in during weekday office hours. After-hours prescriptions are for emergencies only. Please anticipate your prescription refill requirements.
